Grant Submission Form: Use this form every time you submit a grant application. Multiple signatures are required by the College to confirm that you have permission to conduct this grant project if the funds are awarded.

Institutional Review Board (IRB) Policy: Use this link to find information and forms pertaining to the College IRB policy.  An IRB form will be necessary every time you submit a grant application that involves human subjects.

1126 Grant-funded employee request form (fillable pdf form): Use this form to schedule grant-funded personnel.  The 1126 form authorizes a person to work for the number of hours specified.  An 1126 form should be submitted before the employee begins working.  The authorized employee should not work more hours than the 1126 form authorizes.  If you would like an employee to work additional hours for the grant you must submit another 1126 form to authorize the additional hours. .

Time and Effort (T&E) form (word doc): Use this form to certify the number of hours grant-funded personnel work.  All grant-funded personnel authorized to work by an 1126 and who have performed work for the grant must submit a T&E form certifying their effort.  The number of hours that the T&E form certifies should match the number of hours that the 1126 form has authorized the employee to work and hours actually paid as reported by payroll in the Banner system.  The T&E form must be signed by the employee and countersigned by a supervisor familiar with the employee's effort. T&E forms should be collected by project directors at least once per year, but project directors are encouraged to reconcile T&E at the end of each semester.

More forms will be available here soon.

Financial Conflict of Interest: As of 2012, new requirements have been instituted for project directors and senior personnel overseeing federally grant-funded programs. To remain compliant, you must complete training in Research Financial Conflict of Interest once every four years, and you must report 鈥淪ignificant Financial Interests鈥 at least once per year.  Please see below for the required training and reporting form.  Training Module   Conflict of Interest Form
